These Terms are a legally binding agreement between you and Balance Health Concepts & Solutions LLC, doing business as Balance Mind & Body ("we," "us," "our," or "Balance").

Balance Mind & Body offers a range of medical spa and wellness services, including but not limited to aesthetic treatments, injectable services, laser treatments, and wellness therapies ("Services"). All Services are provided by licensed and trained professionals.
We reserve the right to modify, suspend, or discontinue any of our Services at any time, with or without notice.
By enrolling in any Balance Mind & Body membership program, you ("Member") agree to the following terms:
All memberships are entered into a six (6) month minimum commitment. At the conclusion of the initial six-month term, the membership shall automatically renew for successive six-month periods unless the Member provides written notice of cancellation at least thirty (30) days prior to the renewal date. Written notice must be submitted via email, certified mail, or in person to Balance Mind & Body management. Verbal cancellations will not be accepted.
Membership benefits may be shared with one immediate family member only, defined as a spouse, child, parent, grandparent, or sibling. Shared usage must be disclosed at the time of appointment scheduling. Any misuse or sharing outside of this definition may result in immediate termination of the membership without a refund.
All membership benefits, services, units, discounts, and included treatments must be used within the applicable billing or quarterly cycle. Unused benefits do not roll over, carry forward, transfer cash value, or accumulate under any circumstances.
Membership fees are billed automatically to the card on file. The Member authorizes Balance Mind & Body to charge all recurring membership fees and applicable service charges. Failed payments may result in the temporary suspension of membership benefits until resolved. Membership fees are non-refundable, regardless of usage.
All medical treatments provided under a Balance Mind & Body membership are subject to medical clearance and eligibility as determined by the treating provider, as well as clinical appropriateness for each individual patient. Balance Mind & Body reserves the right to modify treatment protocols based on medical necessity. Membership enrollment does not guarantee medical treatment, and all services remain subject to provider approval.
Balance Mind & Body reserves the right to modify membership pricing, benefits, and included services with thirty (30) days written notice. We may also discontinue any membership program with notice while honoring active six-month commitments.
Balance Mind & Body reserves the right to immediately terminate a membership without refund for fraud or misrepresentation, abuse toward staff or providers, violation of medical, safety, or office policies, or improper sharing or misuse of membership benefits.
Memberships are not transferable or assignable beyond the approved immediate family sharing outlined above.
Memberships are not insurance products and do not replace or supplement medical insurance coverage. All services are self-pay unless otherwise stated in writing.
Booking: Appointments can be booked through our Website using our third-party, HIPAA-compliant booking software, Boulevard. By booking an appointment, you agree to provide accurate and complete information.
Cancellation and Rescheduling: We require at least 24 hours notice for any cancellation or rescheduling of an appointment. You may cancel or reschedule by calling our office or using the link provided in your appointment confirmation email.
Late Arrivals: If you arrive late for your appointment, we may need to shorten your treatment time to avoid delaying the next client. The full service fee will still apply.
No-Show Policy: If you fail to show up for your appointment without providing any notice, you may be charged a no-show fee. This fee will be communicated to you at the time of booking.
Payment: Payment for all non-membership Services is due at the time of treatment. We accept major credit cards, debit cards, and other forms of payment as indicated at our facility.
Pricing: Prices for our Services are subject to change without notice. The price you pay will be the price in effect at the time of your appointment.
Refunds: All sales of services and products are final. We do not offer refunds on services rendered. In the event of a defective product, we may offer an exchange for the same product.
